# The Physician Memorial Scholarship Fund

#### The Physician Memorial Scholarship was established in 2008 in memory of the late Bruce C. Urbanc, D.O. It is awarded annually to students who are enrolled or accepted for enrollment in a fully accredited school of medicine or osteopathic medicine. Awards are made to outstanding students who have demonstrated academic excellence.

### Scholarship Information and Guidelines

(Previous applicants need only to provide a school transcript or proof of current enrollment and an updated statement regarding goals, achievements, awards, etc.)

**Dates:**

- Applications will be accepted up to June 30.
- Awards to be given by July 31.

**Award Amount:**

- Minimum $2,000 per annum

**Eligibility Requirements:**

- Currently enrolled or accepted for enrollment in a fully accredited school of medicine or osteopathic medicine.
- A graduate of high school in the area served by Wilson Health.
- A personal statement to include applicant history, interests, activities, achievements, and accomplishments, awards; goals and ambitions.
